Appointment History
1. Appointment
History: This is
specific to the patient’s
appointment history in the system. You can view if the appointments are
canceled or complete in the status
section.
2. Filter: You can view the appointment history
by Upcoming, Past 6 months or the Past 1
year.
3. Check Eligibility: Here you can make sure the patient is eligible to schedule their appointment.
4. Action: Here you can view the appointment details and cancel any appointments.
5. Add An Appointment: Click this button to bring you to the scheduling page.
1. Appointment By Site: This is specific to the patient’s appointment history in the system. You can view if the patient's appointment site, arrival time, the reason, and the meeting type in this section.
2. Site: Here is where you can free text the name of the correct site the patient will be visiting.
3. Date: Here you can search for the appointment date. It will default to today’s date.
4. Search: Use the search tool to search by any word, keyword or phrase to locate any past appointments.
5. Action: Choose Action on any note to edit a re-occurrence, see any required payments or just view the appointment.
6. Add An Appointment: Click this button to bring you to the scheduling page.

Staff Member: Select
the Staff Member you want to schedule
the appointment with.
2. Date:
The date you schedule the appointment to occur, will default to
today’s date.
3. Start Time:
The time you schedule the appointment to begin.
4. Duration: This will allow you to ou can choose the length of the appointment.
5. End Time:
This will automatically populate the end time based on the start time and duration
entered.
6. Appointment Time:
Choose the location the appointment is to be held at.
7. Appointment Reason:
Choose a brief reason for the appointment from the drop down.
8. Transportation: Choose how the patient will be arriving to the site.
9. Appointment Type: Choose the best way to conduct the appointment, by clicking on the correct checkbox.
10. Comment: You can leave additional comments regarding the appointment for the staff member.
11. Client Called: If the patient does not answer the reminder call, then a voice message will be sent to the primary phone number on file.
12. Needs a reminder: Select this checkbox to send a reminder call to the patient in advance.
13. Frequency: Choose the number of times you would like the appointment reminders to occur.
14. End Date: Choose the end date here. It will default to today's date.
15. Submit/Reset/Hide: Here is where you can submit for review, reset the fields and hide the fields.
Cancel Appointments
1. Action: When in the action drop-down select Cancel Appointment. This will prompt a
verification window to confirm your cancellation.
2. No/Yes: Confirm
Cancellation. Select Yes to cancel and you will receive a
success message. You will be able to view the status of the appointment in the
history, to validate that it did cancel.
