Closing a Client in Axiom

There are several reasons to close out a client in Axiom. 

1. The client has ended services with your organization and there EOC (Episode of Care) needs to be closed out

2. The client has a duplicate enrollment in the system and you need to close out the duplicate to ensure that staff are not documenting on two different charts for the client. 

Duplicate Enrollments will typically happen when a client is not being verified as previously enrolled by Front Desk or Registration Staff.

The below process will walk you through how to close out a client in Axiom.




If you do not have access to the closure page, then you most likely are missing the security permissions to view this page. Please reach out to your I.T. or administrative staff to have those permissions granted for your account.


Begin a Closure

On the Closure Page under the Enrollment History Section click on the Action Dropdown and select Action



Enter in the Required Information




Axiom will do a lookup on the client and verify that the client has No Outstanding Notes in the System, the client has no outstanding appointments in the system and others. If they do you will receive a closure Failure Summary view. That will show you all elements needing correction before a closure can be processed.

Closures can be future dated to allow for staff to finish completing notes in the system if needed. Leaving the client opened until the date selected.
