Immunization

Getting There:

The Immunization page can be accessed two ways:
  1. Entering "Immunization" in the form search dropdown.
  2. By navigating to the Axiom page list and selecting "Immunization".
    1. It is located in the "Health" module by default but your organization may have moved it.

Immunization History



  1. Immunization History: This is where you can view all of the patient’s immunization history in the system.
  2. Filter Action: Filters the history table by "Active" or "Expired" status. 
  3. Filter Time: Filters the history table by how recently the entry was created (All, 12 months, 6 months, or single most recent)
  4. Change View: Changes the history from a grid view (as shown) to a card view.
  5. Print List: Prints all entries shown in the history table. The printed list will reflect any filters you have set.
  6. Action: Contains options to "Edit" or "Expire" that immunization as well as "generate a registry submission".



Immunization Registry Settings




  1. Immunization Registry Settings: Collect the patient's vaccination data and save it to the patient's chart
  2. Publicity Code: A code that will let the team know if a reminder or recall is necessary. 
  3. Protection Indicator: A Yes/No option indicating whether the vaccination data should be protected. 
  4. Protection Indicator Date: Choose the date of the Protection Indicator. 
  5. Submit/Reset: Submit will save all fields to the patient's chart. Reset will clear the data from every field without saving.



Immunization Input



  1. Immunization Input: This is where you can enter an immunization to be administered.
  2. Info Source: If this is historical information rather than new information, enter the source of the information.
  3. Vaccine Name: The vaccine being administered.
  4. Administered Amount: The volume of vaccine administered.  
  5. Administered Units: The unit for the volume of the vaccine administered. For example, Milligrams (mg) or Milliliters (mL).
  6. Site: Where on the body the patient received the vaccine.
  7. Route: The route through which the vaccine was given. For example: intranasal or oral.
  8. Vaccine Lot Number: The Vaccine Lot Number from the administered vaccine.
  9. Vaccine Lot Expiration Date: The date of expiration for this lot.
  10. Ordering Provider: The provider that ordered the vaccine. 
  11. Facility: The facility where the vaccine was administered. 
  12. Manufacturer Name: The manufacturer of the vaccine.
  13. No Vaccine Administered: Mark this if the vaccine was not administered. For example: if it was refused by the recipient.
  14. Presumed Immunity: Mark this if the recipient is presumed to have immunity. 
  15. Refusal Reason: The reason provided for refusal of the vaccine. 
  16. Vaccine Financial Code: Vaccine Financial Code (VFC) eligibility status.
  17. Date/Time Start of Vaccine Administration: The date and time of day when the vaccine was administered.
  18. Administered By: The provider who administered the vaccine.
  19. VIS Present to Client Date: When the Vaccine Information Statement (VIS) was provided. 
  20. Comments: Enter additional notes regarding the vaccine.
  21. Submit/Reset: Submit will save all fields to the patient's chart. Reset will clear the data from every field without saving.
