Addresses


  1. Addresses: This will list a history of all records of addresses for the active patient.
  2. Action: You can filter the table by: Current (Default) or by All.
  3. Print List: This will print out the list of addresses shown in the table based upon any filters applied.
  4. Action: You can edit records by simply clicking on the action drop-down.

Address Input




  1. Address Input: This section allows you to input any new addresses that should be associated with the patient.
  2. Address Type: You can select Residential or Billing depending on the Type you are inputting.
  3. Address: Enter the Address and zip code.
  4. City or Zip Code: The zip code will automatically populate: City, State, and County.
  5. Preferred Contact: The drop-down will allow you to select their preferred contact method (Home, Work, Cell #).
  6. Address Status: The drop-down will display, Current, Not Current, Unconfirmed.
  7. Submit / Reset: Once all of the fields have been updated correctly, click Submit. The reset will clear all values.
