Observation Log




Q15 (Observation Log)



  1. Observation Log table: in this table, you will be able to view logs and add new logs for patients. 
  2. Date: this displays the date for which the log is set on. 
  3. Types of log: this shows the three types of logs that can be added such as patient behavior, staff intervention, and staff initials. 
  4. Time: The top of the table will show the time for the log. 
  5. Colored boxes: these will show the observation log that was submitted for that time. Each color represents the type of log submitted. 
  6. Staff initials box: this shows the staff's initials that were submitted for that time. 
  7. Filter Dates: the arrows will take you to the next or previous dates. 
  8. Print Log: this button allows you to print the entire log for the date indicated. 

Adding a Log







Once you click on an empty box an observation log table will open to add any new logs. 

  1. Adding a log: you can add a log by clicking on an empty white box under the applicable time and type of log needed. 
  2. Patient information: this displays the patient's information for which the log is being added.
  3. Log time: log time will automatically default to the one selected when clicking on an empty box in the observation log table. 
  4. Staff initials: initials for staff performing the log can be added here. 
  5. Patient Behavior: you can add this log by selecting from the drop-down menu the type of patient behavior that is observed. The other section is a free text box that allows you to add any other observations not added in the drop-down menu. 
  6. Staff Intervention: you can add this log by selecting from the drop-down menu the type of staff intervention that is needed. The other section is a free text box that allows you to add any other type of staff interventions not added in the drop-down menu. 
  7. Notes: free text box that allows you to enter any additional notes related to the observation log that will be submitted. 
  8. Save/Cancel: this allows you to save and submit the log or cancel.
