Program History
1. Program History: This is the first section on the page, it will display any program history related to the patient.
2. Print List: This will print a list of all program history, showing current and inactive programs. It prints exactly as it looks in the history.
3. Action: Here you can edit the dates of the program, and end or the program
1. Program Input: Here you can add a new program for the active patient.
2. Type: This is where you will select the population for the patient like SMI, GMH and Children etc.
3. Start/End Date: When entering the program it will default to today’s date and you can enter an end date or edit later with the correct end date.
Problem List History
1. Problem List History: Here you can view all active diagnosis codes the patient has been assigned in the system. This will include Behavioral and Physical.
2. Action filter: History will default to show only active problems. You can filter by Active, Inactive or Resolved.
3. Print List: This will print a list of all problems code shown in history. For example, if you are viewing inactive it will print the inactive diagnosis codes only.
4. Action: This drop-down allows you to remove the diagnosis code, edit the problem and the precedence order.
1. Problem List Input: Here you can enter a new diagnosis code to the patient's problem list. You can search by key words, full phrases or by the diagnosis code itself.
2. Problem Description: This allows you to select the correct code needed.
3. Type: When selecting a Behavioral diagnosis code you will see two fill options that allow you to select. This can be Behavioral or Physical.
4. Precedence: If the Behavorial type is selected, a Precedence dropdown will appear. This is how the diagnosis codes are listed in order of importance. 1 being the most important.
5. Condition: This allows you to select if the diagnosis is Chronic or acute.
6. Start Date: This will default to today’s date but can be changed to a previous date when entering. This cannot be changed once you submit.
7. End Date: This section allows you to end date the diagnosis when needed.
8. Comments: This section is an open text box to make any comments related to the clients diagnosis.
9. Submit/Reset: Reset will clear the page of data entered in the input fields. It will keep your search results to select from the list to enter a different diagnosis code. Submit will save the problem in the history.
Behavioral Diagnosis History
This table keeps track of any historical DX codes that are related to the patient. It will also display any updates made by Employees.
