Specialist Referral History



1.   Specialist Referral History: This will list all referral history on the active patient.
2.  Filter: The filter field will allow you to filter the results for: Active, Inactive, or All. 
3.  Search: This is a free-text field, allowing you to search for any text in the referral. Example: Searching PTSD would pull the above result because it found the keyword in the Reason field.
4.  Change View / Print List: Clicking on Change View will adjust the view from a grid view to a card view. Print List will open a printable PDF of the Specialist Referral History.
5.  Action: From here you can edit, expire, view associated files, view, and print.

Specialist Referral Information

Basic Tab


1.    Category / All: Clicking on ALL will change your view from Tab (Basic, Refer to, Submit) to include all fields on a single page (Removing the tabs).
2.    Basic Tab: The active tab will have a dark blue background.
3.    Start Date / End Date: Include the Start and End date for the referral
4.    Requesting Site: Input all of the requesting information, Site, Provider, and Office contact for the referral.
5.    Specialist Type: Clicking this dropdown will provide you with a list of options that you are referring the active patient for.
6.    Max Visits: Indicate the number of Max visits for this referral
7.    Procedure Code: This dropdown will allow you to indicate if this referral is for a Follow-up or for a new evaluation.
8.    Diagnoses: This field will pull from the patients DX Code list.
9.    Authorization Information: Input the correct authorization information and include the authorization number.

Refer To Tab



1.    Category / All: Clicking on ALL will change your view from (Basic, Refer to, Submit) Tab to include all fields on a single page (Removing the tabs).
2.    Refer to Tab: The active tab will have a dark blue background.
3.    Referred To: This is a free-text field, allowing you to input who you are referring the patient to for treatment.
4.    Phone / Fax: Input the correct phone number and Fax number for the referral.
5.    Address / Zip: Enter in the Address and Zip Code for the Facility.
6.    City: This field will automatically perform a lookup on the Zip Code and fill out the correct information.

Submit Tab



1.    Category / All: Clicking on ALL will change your view from Tab (Basic, Refer to, Submit) to include all fields on a single page (Removing the tabs).
2.    Submit Tab: The active tab will have a dark blue background.
3.    Comment / Reason: In this free-text field you can enter any additional comments/reason for referring the patient.
4.    Internal Comment: This free-text field will allow you to create an internal comment that is not attached to the specialist referral.
5.    Submit/Reset: Clicking on Submit will save the Specialist Referral.

Important: After completing a referral you will still need to print it out and fax it to the facility.
